Customer FormsAny of the forms listed below can be downloaded and printed. They have been saved in Portable Document Format (.PDF) files. If you do not currently have software capable of displaying PDF files, you may download a free copy of Adobe Acrobat Reader below.
New Account Forms
Certain types of accounts will require additional forms.
Download Adobe Acrobat Reader
If you do not have Adobe Acrobat (or another program capable of viewing and printing PDF files), click on the icon below. Use your browser's Save As menu selection to save the file to an apporpriate place on your hard drive.
Just follow the instructions provided during the download to install Acrobat Reader on your computer.
Instructions for Printing Forms
If you have installed Acrobat, you are ready to view and download any of the forms above. When you click on the selection of the form you want, it will be displayed on the screen. Use your browser's File /Save As menu selection to save the file. Or, print the forms by clicking on the Printer icon on the Acrobat toolbar which will appear above the displayed forms. Complete the forms per the included instructions. Retain a copy for your records and forward the original, completed forms to Sawtooth Futures. NO FORMS WILL BE ACCEPTED UNLESS ACCOMPANIED BY A COMPLETED BROKER AUTHORIZATION FORM.
